The state runs a four-type medical college system: pure government colleges under DMER, private medical colleges regulated by DMER, deemed universities with their own counselling, and central government institutions like AIIMS Nagpur and AFMC Pune. All MBBS admissions in Maharashtra are conducted by the Directorate of Medical Education and Research (DMER), Maharashtra for 85% state quota seats under Maharashtra University of Health Sciences (MUHS), Nashik.
Maharashtra has 43 government medical colleges offering MBBS at ₹1,62,100 per year for Open category candidates. The flagship trio sits in Mumbai: Seth GS Medical College and KEM Hospital closes under AIR 5,000, Grant Medical College and JJ Hospital, established in 1845, closes under AIR 8,000, and Lokmanya Tilak MC Sion closes under AIR 13,000. BJ Medical College Pune, established in 1964, and GMC Nagpur form the next tier. Newer government colleges in Bhandara, Buldhana, Washim, Jalna, Hingoli and Gadchiroli have noticeably lower cutoffs, opening doors for ranks up to 47,000.
Socially and Educationally Backward Class (SEBC) reservation for the Maratha community is exclusive to Maharashtra DMER counselling and does not exist in MCC national counselling. SEBC candidates get separate cutoff ranks and reserved seats in every Maharashtra medical college. In 2025, the SEBC Round 1 closing rank stood at AIR 47,483 for government colleges and 81,478 for private colleges. Maharashtra also runs VJ (Vimukta Jati), NT-B, NT-C and NT-D (Nomadic Tribes), state-specific categories found nowhere else in India. Maharashtra hosts 52+ private and deemed MBBS colleges. Leading deemed universities include Bharati Vidyapeeth University Medical College Pune, Dr. DY Patil Medical College Pimpri, Mahatma Gandhi Missions Medical Colleges, Rural Medical College Loni (Pravara) and Krishna Institute of Medical Sciences Karad. Private fees range from ₹6,21,500 per year (Dr. NY Tasgaonkar, the cheapest in the state) to ₹15,57,000 per year (Vedantaa Institute, Palghar). All private colleges are regulated by DMER and direct admissions are not permitted anywhere in Maharashtra.
Maharashtra DMER state quota (85% seats) is open only to Maharashtra domicile candidates and uses Maharashtra-specific categories including SEBC, VJ and NT. MCC All India Quota (15% government college seats) is open to candidates from across India and uses national OBC, SC, ST and EWS categories. AIQ cutoffs for Maharashtra's top colleges typically run tighter than state quota since they attract top rankers nationwide.
